The Pendleton School District Facilities will be applying an herbicide treatment across the district to all lawns, ornamentals, and sports fields the week of March 23-27, 2026.

Below are forms that may be downloaded, completed, and brought with you to registration. There may beadditional forms/sign-ups, not found on this website, to complete at registration. Students are welcome to join you.
All students attending SMS for the 2025-26 school must register.

Registration is open for the 2025-26 school year!
To register in person, please bring proof of age and guardianship (birth certificate, hospital or physicians certificate, adoption record, etc.) and immunization records along with the following completed forms:

Birth Certificates
If you need to order a birth certificate and your child was born in Oregon, you may contact the Center for Health Statistics. Instructions for contacting the center are available by visiting the Center for Health Statistics.
If your child was born out of state, you may call information for that state and ask for the Bureau of Vital Statistics. If ordering a birth certificate from another country, please visit the U.S. State Department website.
